Cisco Linksys SPA3000

This device has 1 FXO port (for plugging into the wall), and 1 FXS port (for plugging into a phone).  It is compatible with Asterisk, Trixbox and friends, and speaks SIP. 

This is the older version of the SPA3102, but does the same job.

Product comes in a plain white box.
